Trip to Bhutan - Minimum Daily Tariff inclusion

I am planning to continue my travelling to Bhutan from Nepal but I just found out that travelling to Bhutan is not quite easy. It says that we have a pay a minimum daily tariff * the number of days I spend in Bhutan. I'm wondering if the tour price covers all the basics (visa included)? I'll be arriving from Nepal, from where do the transportation services start for the tour, before/after the border?
Also could any recommend some tour operator contacts

Depending on when you travel, the Government charges USD 250 per person per night or USD 200 per person per night. That will inlcude you accomodation, all meals, transportation during your stay in the country, it will also cover charges for a personalized local guide to take you around the country.

The tariff does not include visa application fee. You will need to make a one time payment of USD 40 to process your visa.

Also if you are travelling alone, for up to 2 people, there is a surcharge of USD 40 per person per night over and above your daily tariff.

The minimum daily package for tourists travelling in a group of 3 persons or more is as follows:
USD 200 per person per night for the months of January, February, June, July, August, and December.
USD 250 per person per night for the months of March, April, May, September, October, and November.

The minimum daily package covers the following services.
• A minimum of 3 star accommodation (4 & 5 star may require an additional premium).
• All meals
• A licensed Bhutanese tour guide for the extent of your stay
• All internal transport (excluding internal flights)
• Camping equipment for trekking tours

It also includes:
• All internal taxes and charges
• A sustainable development fee of $65. This fee goes towards free education, free healthcare, poverty alleviation, along with the building of infrastructure.

Individual traveller upto 2 people are subject to an addtional surcharge of USD 40 per night over and above the minimum daily tariff.
